Your First Appointment

 

Generally your first appointment will average 50 to 90 minutes.  After that most appointments are about an hour.  You can download and print out the forms below if you would like to save time during our session.  These are basic forms that give us more information about one another.  Please feel free to come to your appointment with questions about my background, qualifications and whatever else would make you feel more comfortable.

 

Payment

 

My fee for service is $180 per hour for individuals and $200 for families and couples.  I am on most insurance panels.  You may want to check before you come and see me to make sure I am on your insurance panel and to find out what your co-payment will be.  You can do that by calling the number on the back of your insurance card.

Payment is due at time of service or by arrangement.  I take cash, checks and credit cards.

I offer reduced fees for people in need who do not carry insurance.

Good Faith Estimate

You have the right to receive a “Good Faith Estimate” explaining how much your therapy might cost. Under the law, we need to give people who do not have insurance or who are not using insurance an estimate of the total fees for their therapy per calendar year. You can ask for a written Good Faith Estimate before you schedule a session, before your first appointment or at any time while you are receiving services.  

If you receive a bill that is at least $400 more than your Good Faith Estimate, you can dispute the bill.  Make sure to save a copy or picture of your Good Faith Estimate.
